Abstract

INTRODUZIONE: L'insufficienza renale acuta è una condizione che si verifica frequentemente nelle unità di terapia intensiva e richiede un trattamento mirato. Il team infermieristico deve essere adeguatamente formato per gestire i pazienti sottoposti a terapia renale sostitutiva continua. 


OBIETTIVO: Sviluppare e validare il Continuous Renal Replacement Therapy - Measurement Competency Tool (CRRT-MCT), volto a misurare la competenza infermieristica nella gestione dei pazienti sottoposti a tecniche di sostituzione renale continua. 


DESIGN: Studio trasversale di validazione dello strumento con test-retest. 


PARTECIPANTI: 30 infermieri di area critica hanno partecipato allo studio. 


METODI: Per la creazione dello strumento sono state prese come riferimento le attuali linee guida per le tecniche di terapia renale sostitutiva continua. Un gruppo di 10 esperti ha valutato il nuovo strumento tramite un sondaggio online. Sono state misurate la validità di contenuto e di facciata dello strumento. L'affidabilità dello strumento è stata indagata attraverso il test-retest e calcolando l’indice di correlazione di Pearson; l'α di Cronbach è stato invece calcolato per valutarne la consistenza interna. 


RISULTATI: Sono stati condotti test pilota e test-retest con 30 infermieri di area critica. Lo strumento CRRT-MCT ha ottenuto una buona validità di contenuto e di facciata (S-CVI= 0,97; I-CVI=87%-100%) cosi come una buona consistenza interna (α di Cronbach= 0,799). La correlazione intraclasse per l'analisi test-retest ha evidenziato un'eccellente affidabilità test-retest, confermando la stabilità dello strumento. 


CONCLUSIONI: Lo strumento misura la competenza degli infermieri di terapia intensiva relativamente alle tecniche di terapia renale sostitutiva continua e si è dimostrato valido e affidabile. le esigenze ambulatoriali dei pazienti nella “nuova normalità”, sfruttando i nuovi strumenti tecnologici a disposizione del Servizio Sanitario.
