Abstract

INTRODUZIONE: L’insufficienza cardiaca, ha un enorme impatto in termini di mortalità, morbilità e costi per le organizzazioni sanitarie. Le indicazioni per l'impianto di dispositivi cardiaci, come pacemaker o defibrillatore cardiaco impiantabile (ICD) sono in costante aumento. Con l’arrivo della telemedicina, gli infermieri giocano un ruolo importante nella gestione a distanza delle malattie cardiache croniche. Lo scopo di questo studio è descrivere le esperienze degli infermieri, che monitorano a distanza i pazienti con un dispositivo cardiaco impiantato, in Italia.
METODO: È stato condotto uno studio qualitativo con un approccio fenomenologico. Il campione era composto da 7 infermieri dedicati al monitoraggio in remoto (RM), provenienti da tre Ospedali del Nord Italia. Abbiamo condotto interviste semi-strutturate per comprendere l'esperienza e la formazione che gli infermieri hanno ricevuto per condurre il RM.
RISULTATI: Dall'analisi delle interviste, sono stati identificati sei temi: 'Conoscenze e competenze specialistiche', 'Apprendimento attraverso la pratica', 'Organizzazione del lavoro', 'Processo decisionale', 'Fiducia', e 'Bisogno di informazioni'. La maggior parte del nostro campione ha sviluppato le proprie competenze direttamente sul campo, in assenza di un percorso certificato e riconosciuto, sul RM nei pazienti con scompenso cardiaco impiantati di device cardiaco. Pertanto, gli infermieri che svolgono attività di RM non ricevono lo stesso tipo di istruzione e formazione.
CONCLUSIONI: Attualmente, in Italia, non esiste una formazione standardizzata per gli infermieri che svolgono attività di RM, eppure è fondamentale avere conoscenze e formazione adeguate a poter svolgere efficacemente questo tipo di attività e assistenza.
