Abstract

RIASSUNTO

SCOPO: individuare l'orientamento della ricerca infermieristica italiana nella pratica assistenziale evidenziandone punti di forza, eventuali necessità di miglioramento, suggerimenti e raccomandazioni per aumentare lo sviluppo della conoscenza.
METODO: è stata condotta una revisione estensiva della letteratura (scoping review) consultando le seguenti banche dati: PubMed, CINAHL, PsycINFO, Biosis e Scopus, nell'intervallo di tempo compreso tra gennaio 2006 e luglio 2016.

RISULTATI: Su un totale di 5635 pubblicazioni, 35 sono state individuate come pertinenti al quesito di ricerca. Queste evidenziano l'evoluzione e i cambiamenti messi in atto dagli infermieri in Italia nel proprio agire professionale in funzione delle migliori evidenze disponibili soprattutto a partire dal 2011/2012. Molte pubblicazioni riguardano l'implementazione di nuove procedure tecniche o sistemi educativi per il paziente, mancano invece studi finalizzati a dimostrare l'efficacia delle cure infermie- ristiche e i loro effetti sui pazienti. Molti studi non appartengono a filoni di ricerca o a progetti piuÌ€ ampi, cosiÌ€ come quasi nessuno ha visto la partecipazione di gruppi di ricerca internazionali. Per quanto riguarda gli ambienti clinici in cui sono stati condotti gli studi analizzati, la maggior parte sono rappre- sentati da realtaÌ€ ospedaliere e l'attenzione è stata rivolta principalmente a pazienti in fase acuta di malattia; pochi studi hanno indagato invece soggetti affetti da patologie croniche, anziani o sono stati condotti in realtaÌ€ assistenziali quali le case di riposo. Altro elemento che si puoÌ€ rilevare dalla presente revisione è la prevalenza di studi quantitativi, con i disegni di studio descrittivo, utilizzo di questionari molto spesso creati ad hoc e l'arruolamento di campioni di popolazione di convenienza. Quanto rile- vato indica la necessitaÌ€ di rivedere gli orientamenti della ricerca infermieristica italiana, ponendo l'at- tenzione sulle prioritaÌ€ emergenti a livello clinico e assistenziale. Tutto questo dovrebbe avvenire garan- tendo una sorta di compromesso tra le esigenze degli infermieri e i bisogni assistenziali dei pazienti.

CONCLUSIONI: gli infermieri italiani nella ricerca rivolta alla pratica assistenziale prediligono temi legati ad aspetti tecnici ed educativi. Manca quasi completamente negli studi la parte teorica o un framework filosofico/teorico di riferimento, suggerendo la necessitaÌ€ di condurre nuovi studi e appro- fondimenti. I ricercatori italiani nella maggior parte dei casi non sembrano seguire dei filoni dettati da prioritaÌ€ di ricerca, indicando di conseguenza, l'esigenza di rivedere gli ambiti assistenziali che neces- sitano di essere sviluppati, esplorati o consolidati. Per gli infermieri italiani la ricerca è quindi un ambito ancora in fase di sviluppo e consolidamento, ma con forti potenzialitaÌ€ per il futuro.

ABSTRACT

PURPOSE: To identify orientations in Italian nursing research regarding nursing practice, highlighting strengths, needs for improvement, as well as suggestions and recommendations for promoting the future development of nursing knowledge.
METHOD: An extensive review of the literature was conducted (scoping review) by consulting the following databases: PubMed, CINAHL, PsycINFO, Biosis and Scopus, for the time interval between January 2006 and July 2016.

RESULTS: Out of a total of 5635 publications, 35 were identified as relevant to the research question. These highlighted the changes implemented by nurses in Italy through their professional actions according to the best available evidence, especially since 2011/2012. Many publications relate to the implementation of new technical procedures or educational systems for patients, but there is a lack of studies that examine the effectiveness of nursing care and its effects on patients. Many studies do not belong within a program of research or larger projects, and virtually none involve participation in inter- national research teams. With regard to the clinical environments in which the studies were conducted, the majority involved hospitals and the focus was mainly on patients in an acute phase of an illness. Few studies have investigated persons with chronic illnesses, the elderly or have been conducted in long-term care settings such as nursing homes. Another finding identified in this review is the prevalence of quantitative studies, with descriptive study designs, the use of questionnaires that were often created ad hoc, and the recruitment of samples from "˜convenience' populations. These findings highlight the need to further review orientations in the Italian nursing research literature, focusing attention on emerging clinical priorities. This should be done through a process that balances the needs of nurses and with those the patients.

CONCLUSIONS: Italian nursing researchers tend to focus their investigations on technical and educa- tional topics. Theoretical or philosophical frameworks are almost completely missing in the literature, highlighting the need for more work in this area. In most cases, Italian researchers do not seem to follow research tracks based on research priorities, highlighting the need to review practice settings to identify areas that need to be developed, explored or consolidated. For Italian nurses, research is an area that is still under development and consolidation, but with strong potential for the future.
