Abstract

Obiettivo: Analizzare il livello di complessití  assistenziale e la sua relazione con la mortalití  e la durata della degenza.

Metodo: Studio prospettico osservazionale effettuato nel periodo 1/06/2014 - 21/06/2015 presso la Medicina d'Urgenza di un'Azienda Ospedaliero-Universitaria italiana. Sono stati rilevati giornalmente i bisogni di assistenza infermieristica secondo il Modello delle Prestazioni Infermieristiche e l'Indice di Complessití  Assistenziale (ICA).

Risultati: Sono stati inclusi 506 pazienti per un massimo di 4 giornate di degenza (1.436 rilevazioni). Le prestazioni con il valore più alto sono state soprattutto finalizzate ad assicurare la funzione cardiocircolatoria. L'ICA medio è stato di 38,4. Solamente il 17,2% dei pazienti è stato classificato ad alta complessití  nel giorno d'ingresso. Nel corso della degenza, l'ICA ha dimostrato una progressiva tendenza alla diminuzione nei soggetti sopravvissuti e all'aumento in quelli deceduti. L'accuratezza predittiva dell'ICA rispetto al rischio di morte è risultata complessivamente elevata (90,3%; p=0,006); il migliore livello di cut-off è risultato pari a 41,5 punti.

Discussione: È stato evidenziato che il rischio di morte aumenta all'aumentare della complessití  assistenziale. Il superamento del valore di cut-off, cosí¬ come una tendenza all'aumento dell'ICA nel monitoraggio quotidiano, potrebbero essere adottati come criteri per identificare i pazienti a più elevato rischio, per i quali aumentare, ad esempio, il rapporto infermiere-paziente. L'identificazione del livello di complessití  assistenziale può consentire l'erogazione di prestazioni differenziate in base all'intensití  delle cure e la corretta allocazione dei pazienti durante il ricovero. Tale strategia può consentire la razionalizzazione delle risorse disponibili e una migliore pianificazione e attuazione dell'assistenza. Distribution and predictive accuracy of the Index of Caring Complexity in a semi-intensive care unit

Objective: To analyze nursing complexity level and its relationship with mortality and length of stay.

Method: Observational study carried out between 1/06/2014 to 21/06/2015 at High Dependency Unit of an Italian University Hospital. The nursing needs and the Index of Caring Complexity (Indice di Complessití  Assistenziale, ICA) were collected following the "Modello delle Prestazioni Infermieristiche".

Results: Data from 506 patients were collected for a maximum of 4 days of hospital stay (1,436 data collection). Nursing needs with the higher score were mostly related to cardio-circulatory function. Average ICA was 38.4. Only 17.2% of patients were classified as "high complexity" at admission. During the hospital stay, the ICA showed a progressive decrease in survivors while increased in those who died. Overall, the ICA showed high predictive accuracy regarding the risk of death (90.3%; p=0.006); the best ICA cut-off point was 41.5. Discussion: The risk of death increased with the increasing complexity of nursing care. Exceeding the ICA cut-off value, as well as an increasing trend in the ICA daily monitoring, may be adopted as criteria to identify patients at higher risk, for which increasing, for example, the nurse-patients ratio. Identification of the patient's complexity level may enable the delivery of more personalized nursing activities or interventions based on the intensity of care needed, as well as the proper allocation of patients during hospitalization. This strategy may allow the rationalization of available resources and better planning and implementation of nursing care.
